This is not a final list but will allow us to be prepared for when the final underwriting guidelines are released.  We expect these guidelines to be released on or before Monday April 13th.  We do not anticipate needing much beyond this list.  If we do need additional documents uploaded, you’ll receive an email with detailed instructions.

Once your CaresLoansASAP Registraion is complete, you will be directed to our secure portal and asked to upload and/or electronically sign the following documents.

Execute Disclosure Document with e-signature. This document explains that there is no fee for our service, and you will receive 100% of your loan amount with absolutely no fees for the service but we may accept referral payments from the banks or credit union that funds your loan.


  • Upload Articles of Organization/Incorporation
  • Upload Operating Agreement or Corporate By-Laws
  • Upload Copy of Business License (if applicable)
  • Upload Copy of Driver’s License for all people that own 20% or more of the company


  • Upload most recent available Tax Return
  • Upload most recent available trailing 12 month profit and loss statement (2019 is fine)
  • Upload 2019 and YTD Payroll Summary Report/Register including benefits (vacation, severance, benefits, etc.)
  • Upload 2019 Quarterly IRS Form 940, 941 and 944 payroll tax reports (4 total)
  • Execute Paycheck Protection Program Application

Once all of your documents are complete, our goal is to send you the name of the bank or credit union that we have selected to underwrite your loan within 5 business days. You will be updated throughout the process via email and in your secure portal.
